Important Notice
For purposes of this agreement, “Blue Chip” refers collectively to Blue Chip Elite, Blue Chip Basketball Association, and Bluechip Athletix, LLC, including their owners, officers, employees, agents, volunteers, and affiliates.

Acceptance of this agreement may be executed electronically and is legally binding upon submission of any registration, tryout, or participation form.

SMS / Mobile Messaging Terms

Blue Chip may send SMS/MMS messages to athletes, parents/guardians, coaches, directors, staff, and program contacts for customer-care and operational communication.

Messages may include BCOS access links, schedule updates, IDP membership updates, tournament availability reminders, roster or activity updates, urgent operational alerts, issue-report follow-up, and replies to inbound support requests.

Message frequency varies based on your participation, schedule, and communication needs. Message and data rates may apply.

You can opt out at any time by replying STOP. You can request help by replying HELP or by contacting Blue Chip at info@bluechipelite.com or 904-395-5527.

Consent to receive SMS/MMS messages is not a condition of purchasing any product or service.

Mobile opt-in data and text messaging consent will not be shared with third parties or affiliates for marketing or promotional purposes.

Blue Chip may use service providers, such as messaging and communications platforms, only as needed to deliver and support these communications.

Medical & Activity Liability Waiver

I, the undersigned participant and/or parent or legal guardian, acknowledge, agree, and understand the following:

  • Participation in Blue Chip activities is voluntary and undertaken of my own free will.
  • I affirm that the participant is in good health and proper physical condition to participate.
  • I understand that participation in sports involves inherent risks, including serious injury or death.
  • These risks include, but are not limited to:
    • Running, jumping, stretching, sliding, or diving
    • Collisions with other participants or stationary objects
    • Playing conditions, facilities, and equipment

I knowingly and voluntarily assume all risks of injury, illness, or death arising from participation in Blue Chip activities, including while:

  • Practicing or playing as a team member
  • Participating in non-playing team activities
  • Being present at any facility or premises used for Blue Chip activities

Release of Liability

I hereby release, discharge, and agree not to sue Blue Chip for any claims, damages, losses, or causes of action arising from participation, including those resulting from negligence, breach of contract, or wrongful conduct, to the fullest extent permitted by law.

Communicable Disease Acknowledgement

I acknowledge the risk of exposure to communicable diseases, including but not limited to COVID-19, and understand that it is impossible to eliminate all risk of infection while participating in group activities.

Parent / Guardian Consent & Indemnification (Minors)

For participants under the age of 18:

I certify that I am the parent or legal guardian of the participant and consent to their participation. I further agree to indemnify, defend, and hold harmless Blue Chip from any claims arising from my minor child’s participation, even if arising from negligence, to the fullest extent permitted by law.

Media / Model Release

I grant Blue Chip permission to photograph and/or record video of the participant during activities and events.

These images and recordings may be used for promotional materials, websites, social media, publications, and broadcast or electronic media, including but not limited to Hudl, BallerTV, and MaxPreps.

I understand that no compensation will be provided and that such media becomes the property of Blue Chip.
